Dextroposition is a rare cardiac malformation defined as rightward displacement with leftward apex, vs. dextrocardia, which base-axis orientation. We report the second known case of dextroposition accessory pathway, presenting preexcited atrial tachycardia. Abnormal anatomy complicates ECG localization and pathway ablation.
